This year, the National Association of Fraternal Insurance Counselors (NAFIC) will hold their annual meeting in Florida at Disney World! The dates of this meeting will be May 28, 29, and 30th. The theme of the meeting is "Fraternalism and the Family." Featured speakers will include Dr. Kerry Johnson, Jeanne Robertson, Jeff Slutsky, and W. Mitchell. You may get the latest details at www.nafic.org. I can personally assure you that this will be the best sales meeting you will attend this year, with no exceptions!

FCSLA wants our producers to attend this meeting, and so we are making this very special offer. If a producer places $12,000 of annualized life insurance premium (credit for single premium will be 5% of the total) by May 10, 2008, we will pay for the plane ticket, the hotel stay (three nights) and the cost of the meeting attendance.

As you know, FCSLA strongly endorses the FIC program, and the Association reimburses all course and testing fees for our producers who work for and obtain the FIC and FICF designations. Our producers may attend the NAFIC annual meeting whether or not they currently have these designations or are even enrolled. Our purpose in recommending attendance is the exposure to this outstanding program and the sales ideas that come from it. We hope our producers obtain one of these designations in the future. Non-Conforming Illustration Form (39K PDF*) - This form must be submitted with any life insurance application that is not accompanied by a signed illustration matching the application.

Life Insurance Underwriting Guidelines Chart (11K PDF*) - This document shows the medical information we require for applicants based on issue age and amount of insurance applied for.

Suitability Form Requirements (48K PDF*) - This document explains the suitability form requirements by state.

Annuity Suitability Form (14K PDF*) - This form must be submitted with any annuity application where required by the state.

Life Insurance Suitability Form (14K PDF*) - This form must be submitted with any life insurance application where required by the state.

Replacement of Life Insurance or Annuities Form (54K PDF*) - This form must be submitted with any replacement application (life insurance or annuity). One copy to the Home Office with application, one copy to applicant, one copy to agent.

Sales Material Verification Form (37K PDF*) - This form must be submitted with any replacement application (life insurance or annuity). One copy to the Home Office with application, one copy to applicant, one copy to agent.

Nonqualified Section 1035 Tax Free Exchange Request (35K PDF*) - This form is required if the applicant is surrendering a policy with another company and using that surrender as a tax-free exchange to purchase a policy from FCSLA. This form must be submitted with the Replacement of Life Insurance or Annuities form.

HIPAA Authorization (97K PDF*) - This authorization will be used to obtain medical records, if necessary, from the doctor(s) listed on the back of the form.

Third Party Delivery Permission (25K PDF*) - To be completed and signed by the applicant if he wishes to have the certificate delivered to someone other than the certificate owner.
